A Clinical Investigation of Abdominal Trauma by Cultivator

Abstract
A clinical investigation of 42 cases of abdominal trauma by cultivator who underwent explore laparotomy at Chonnam University Hospital during the past 5 years, from Jan., 1977 to Dec., 1982 was performed.
After clinical review, following results were obtained.
1) The sex distribution was 31 males(73.8%) and 11 females(26.2%) with ratio 2.8:1 and 4th and 5th decade(69%) showed highest incidence.
2) The seasonal distribution showed the highest incidence in summer involving l4cases(33.3%) and the most of accidents developed in the daytime.
3) The most common cause of accident was unskilled operation in 29 cases (69.1%), and the majority of an injuried person was operator in 28 cases(66.7%), and the handle of cultivator was frequent cause of abdominal injury in 19 cases (45.2%).
4) The clinical manifestations included abdominal pain in 90.5%, abdominal tenderness, 85.7%, abdominal rigidity, 83.8%, rebound tenderness, 78.6%, and abdominal distention, 59.2%.
5) 35 cases (83.3%) out of 42 cases in which abdominal paracentesis was performed had a positive finding and in peritoneal lavage 6(85.7%) out of 7 cases were positive.
6) The involved intra-abdominal organs, in order of frequentcy, was spleen(29.4%), small bowel(23.5%), mesentery(l5.7%), and liver(13.7%), and 8 cases(19%) had multiple organ involvement.
7) The frequently associated extra-abdominal injuries were chest injuries (3 cases), head and neck trauma (2 cases) and upper extremity trauma (2 cases).
8) The cases who admitted within 6 hours from injury were 23 cases(55.1%) and the majority of cases(90.9%) within 12 hours.
9) In small bowel injuries simple closure or resection and end to end anastomosis, in liver injuries simple closure or resection, and in spleen injuries splenectomy or splenorrhaphy was performed.
10) The postoperative complications were 4 cases (9.5%), and they were wound infection, bowel adhesion, intra-abdominal abscess and pulmonary complication.
11) There was no death in the cultivator accident probably due to less serious injury than the other traffic accident.
